POLICIES AND PROCEDURES

 

OVERVIEW OF PROCESS TO RECEIVE STUDENT ACCOMODATIONS 

  • Students must self-identify with the Disability Services Coordinator and formally request accommodations by completing the  and providing any supporting medical documentation. Accommodations are not retroactive, and the University is not obligated to provide accommodations until the application process has been completed and accommodations have been officially approved.
  • Students must meet with the Disability Services Coordinator and engage in the interactive process, which involves a discussion of the accommodations.
  • Students must meet with the instructor to discuss the approved academic accommodations.
  • Students must renew their accommodation request EACH SEMESTER by completing the .

 

PREGNANT AND PARENTING STUDENTS

The Disability Services Policy covers pregnant and parenting. Additionally, students seeking adjustments due to pregnant and parenting status must complete the .


FORMS

This form is for students to apply for academic accommodations.

This form is to be completed by the medical provider. The Medical Documentation Guidance explains the required medical information that is needed for accessing accommodation needs.

This form is for formal grievances by students for disability discrimination under the Disability Services Policy. This includes disagreements regarding a requested service, accommodation, auxiliary aid, or modification of a System practice or requirement; denial of a request; inaccessibility of a program or activity; or a violation of privacy in the context of a disability.
